A 53-year-old AA male presents to the ER four days this week. He c/o foul odor and drainage of R foot. States that he continues to return to ER because he has no signs of getting better. To reduce instances of the patient returning to ER, the APN expects to gain trust by using precise and straightforward communication congruent with the patient’s singularity to form a collaborative relationship to devise a collaborative plan of care and patient healing resulting in patient satisfaction. The patient and APN came to a reciprocal agreement which included verbalizing his understanding of proper antibiotic use, the importance of wound care, and NWB orders, provided and the APN. APN also ensures the patient that he can contact her during office hours with any questions or concerns.
