Mental health requires strategic measures and interventions that improve individuals'

wellbeing, including exposure to the right information, resources, and environment. For instance,

the focus is on giving therapies to patients with mental health and improves their understanding

of the condition and self-care initiatives. Through the patient-centred care model, such patients

can improve their current situation by being objective and positive about their condition (Jones et

al., 2018). Moreover, identifying the consistent risk factors such as misinformation, inconsistent

cultural and social practices, lack of optimal environment, and other situations have to be

addressed.

Another goal is on including the family and other community stakeholders in addressing

the health risk factors and inducing the patients to caregiving models that suit their

understanding. The preliminary care coordination plan seeks to identify the most consistent risk

factors, including within the home, workplace, or community environments. It also identifies the

potential community resources that can be used to address the problem within the desired

frameworks (Lyon & Bruns, 2019). The nurses and other care providers have to identify the

models that can enhance the management of mental health issues. It is achieved through holistic

healthcare practice, including family and community resources in an integrated framework.

Community Resources

A safe and effective continuum of care requires that patients be exposed to and informed

about their communities' mental health resources. These include governmental and NGO

programs that seek to improve their wellbeing and wellness. Each of the resources used has

impacts on the patients' health and wellbeing, including introducing them to the community of

mental health individuals. Moreover, the patients are exposed to mental health service providers,

programs, and resources within their communities (Enos, 2017). For instance, it is right to

expose them to non-profit organizations that offer mental health therapy and management

services. Through such organisations, the community outreach initiatives enhance resource

access, coordination of activities and create a network that helps with mental health outcomes.

Besides, local services within the community and support groups are integral resources

that expose the patients to advanced resources and services to help them with knowledge and

information. The patients also get clinical trial and online resources that enhance their

information access and utilization. They can compare the impacts of their conditions with others,

including the nearest therapies available, inpatient and hospital care, and access the virtual and

digital databases such as Psychology Today, National Mental Health Groups and Associations

(NAMI) National Coalition for Mental Health Recovery. Moreover, they are also exposed to

online texting and messaging or phone services to achieve the undersigned roles.
