As an acute clinician nurse we perform dialysis treatments on patients in the hospital setting. We utilize primary team care model and it allows us to provide optimal direct highest care to our clients with great level of autonomy and quality. As part of a team care we collaborate together with primary nurse, nephrologists and other interdisciplinary teams to provide best quality of care to maximize optimal clinical outcomes. Moreover, providing care based on Compassion, Advocacy, Resilience, and founded on Evidence-Based Practice will highlight patient-centered care and improve collaboration with others. One of the improvements we can accomplish is our clinical setting is to be able to contribute in assisting patients in process of continuity of care and transition of the dialysis treatments from acute hospital to outpatient chronic setting. .Compassion Compassionate care with collaboration is essential in nursing practice. It is not just related to patient’s clinical outcome and satisfaction, but also enhances alliance with other interprofessional multidisciplinary teams. It creates synchronization of workflow and safe work environment. Wedemonstrate care and compassion to our patients and their family members through building therapeutic relationship, active listening and addressing their concerns. Moreover, provide them more information with re-assurance will ease them during treatment and improve clinical outcomes with compliance and regiment adherence
