These conditions are role definition, the presence of direction or purpose, a framework or structure within which the team’s processes or activities are carried out, access to resources, and a coach or a mentor within the team (Cavanaugh et al., 2021). Enabling these conditions within the interdisciplinary team will foster teamwork and enhance their effectiveness in coordinating care for their patients.

The team’s purpose is to provide quality care that optimizes clinical outcomes. This purpose will be communicated to the team. The team members will be expected to work diligently towards optimizing care outcomes. The plan is expected to be generally acceptable to the team members as it doesn’t have many considerations.

Change Theory

Kotter’s change management theory is an example of a change theory that can help interdisciplinary team members buy into the plan. This change theory casts the responsibility of managing change on the organizational leader. This theory defines an 8-step process for implementing changes within an organization. Kotter’s change theory is based on the fact that people tend to resist change. The theory, therefore, outlines steps of tailoring the team members’ psychology toward accepting the change process (Carman et al., 2019).
