ID: 50 Y.O Caucasian male who works as a high school English teacher. CC: Cough/fever/green, blood-tinged sputum HPI: Mr. A presents to the clinic with cough for the last 10 days that is getting worse. Reports coughing up green sputum on occasion and the last two days he has coughed up blood-tinged sputum. Cough is worse in the morning and comes from “deep down” in his chest and sometimes getting long coughing spells that keep bringing mucous up. Past Medical History: Cut cornea as a child (no current issues, but wears glasses and is seen annually). Braces placed recently and cleanings done every 6 months. Past Surgical History: Tonsils and adenoids removed as a child. Inguinal hernia repair done in 2012. Family History: Mother is living and has history of hypertension and addiction. Father passed in 2021 from pancreatic cancer. Maternal Grandmother passed in 2001 from COPD and was lifelong smoker. Maternal Grandfather is unknown. Paternal Grandmother is living and has diabetes, Crohn’s disease, and fibromyalgia. Paternal Grandfather passed in 2020 from cancer of unknown etiology. Brother is living and has history of addiction, and HIV. Social: Smoking since age 21. Currently not ready to quit. Denies illicit drug use or alcohol use. Medications: OTC Robitussin and Tylenol PRN for cold s/s. Allergies: No known allergies. ROS: General – Positive for fatigue, body aches, chills, night sweats, shortness of breath. Fevers noted since the cough started with highest being 100.3. Lives with wife and dogs and feels safe at home. HEENT: Headaches off and on for the last 5 days note. Pain rated 5/10 that is relieved by Tylenol and pain goes to a 2-3/10. Right ear pain noted when coughing. Ear currently not in pain. Denies runny nose, watery eyes, swollen/painful glands, congestion. Resp: See above
