    This is about a female patient, my grandmother, and my mother. They are my patients. My grandmother previously had health issues, including a heart attack and a stroke, which have left her with mobility and vision issues. She has had chronic hypertension related to nonadherence to her medication regimens and lifestyle habits.

    Patient Family

    My mother, who is also her caregiver, struggles with hypertension as well, is pre-diabetic, has insomnia, and has experienced caregiver burnout for years. They both lack consistency, urgency, and discipline to follow recommendations to manage their disease process successfully. Over time, this has led to the worsening of the condition with other complications stemming from this disease.

    Population Health Problem Solution

    My goal with this capstone project is to effectively bring awareness to the issue and create an intervention to inspire change. The desired outcome would be for the patients to accept the need for a change and make a consistent effort towards improving their lifestyle habits for the change to be effective and continuous as an intervention. I have created a brochure covering the primary categories that, if modified, will significantly impact the patient’s life. These categories include medication adherence, the DASH diet, exercise, stress management, fall prevention, and community resources available for the patient and caregiver.

    Implementation of Evidence

    We have had multiple meetings over the past few weeks. I tried to teach in small doses over multiple times in a day or multiple interactions every day. The meetings were primarily held in the patient’s house, on the go, in the car, at the store, or in the hospital—wherever the patient was. By providing microdoses of information, I noticed better retention and less overwhelming responses from the caregiver and patient.

    For example, if we were taking my grandmother’s blood pressure and it was high, I would provide immediate education based on guidance from the American Heart Association and the CDC. Topics included proper positioning for blood pressure measurements, medication adherence, and stress management techniques.

    Barriers to Implementation

    When presenting the intervention to the family, the caregiver agreed that this was an issue and recognized the adverse effects of disease mismanagement. However, cultural norms, caregiver fatigue, and environmental disruptions posed significant barriers to implementation. It required consistent reinforcement and collaboration to overcome these challenges.

    Leadership Strategies and Change Management

    By applying leadership strategies such as clear communication, patience, and adaptability, I was able to engage the caregiver and patient. My approach included aligning interventions with the patient’s daily activities, ensuring manageable adjustments, and emphasizing the long-term benefits of these changes.

    Cost Considerations

    The implementation of these strategies was cost-effective as it relied on readily available community resources, such as those provided by the CDC and local health departments. This ensured that financial barriers did not impede progress.

    Management and Policy Considerations

    Through this experience, I recognized the importance of policies and guidelines that support patient education and consistent follow-up. Patients often face gaps in communication and collaboration among healthcare professionals, leading to inadequate understanding and adherence to care plans. Policies should ensure that all medical healthcare professionals provide culturally sensitive care.

    Practices and Patient Outcomes

    My intervention included creating a medication calendar, engaging the caregiver in stress management activities, and utilizing community resources such as Meals on Wheels and transportation services. This holistic approach enhanced patient safety, improved adherence, and fostered a positive environment for sustainable change.
