Part Two Contribution of the Intervention to Patient or Family Satisfaction and Quality of Life The patients were actively involved in the assessment process, and were willing to change their behaviors, to effectively manage their conditions. Similarly, each patient took part in the education, which was a key part of patient care, stressing the need for patient compliance. They also reported that using the telehealth acted as a reminder to take their medications on time, and motivated them to constantly adhere to their interventions. In addition, these patients adopted a wide range of self management strategies ranging from physical exercise and healthy diet to self-care, such as foot care for diabetics with foot ulcers. As a result, the general health if the elderly patients with chronic illnesses was greatly improved. The number of hospital readmissions resulting from noncompliance was reduced by at least 80%, showing the improvement of the patients' quality of life as well as achieving positive health outcomes. As there was effective communication between the patients and their healthcare providers, the patients' feedback revealed their improved experience along with satisfaction, leading to high rates of adherence to medications and maintenance of self management measures.
