Patient-Centered Care in Practice

As a nurse, I am a key member of the interdisciplinary team. I am involved in patient education, monitoring, formulating nursing care plans, and administering safe medication. As such, I am committed to promoting quality healthcare services. To accomplish this, I work in concert with physicians, nutritionists, pharmacists, physiotherapists, and laboratory technicians.

According to Li et al. (2019), evidence-based practice (EBP) refers to a process of reviewing, analyzing, and translating current scientific evidence. EBP aims to leverage the best current evidence and clinical expertise to improve patient care (Li et al., 2019). Also, EBP incorporates patients’ preferences to achieve patient-centeredness and holistic care (Li et al., 2019). For instance, psychotherapy can be used to manage depression in patients who are not willing to take medication. On the other hand, quality improvement (QI) refers to the conceptual structure used to optimize care (CMS.gov 2021). To achieve this, QI embraces standardized structures and procedures to create uniformity and minimize variations in results (CMS.gov 2021). A quality improvement initiative that focuses on cultural sensitivity can be used to achieve patient-centered care. Lastly, informatics entails integrating data, information, and human knowledge to improve the quality of care and health (AMIA.org, n.d.). Informatics promotes patient-centered care by allowing healthcare providers to collect and analyze patient data to make individualized clinical decisions.

I have professional experience using QI to improve safety in the workplace. The QI initiative aimed to minimize the number of falls at the hospital. The initiative focused on identifying and cleaning spillages promptly, fixing non-slip floors in high-risk areas, and screening patients using a standardized tool. The QI initiative reduced monthly cases of falls by 95 percent.

The interdisciplinary team should leverage QI, informatics, and EBP to improve patient safety and the quality of care. The team should embrace open communication, set goals and objectives, and uphold performance evaluation to accomplish this. Essentially, these strategies will promote appropriate delegation of tasks, care coordination, and accountability among team members. Conclusively, nurse informaticists will play a key role in integrating informatics into clinical practice.
