Summary of Research Findings and Their Relevance to the Project’s Outcome

These research findings are centered on the thematic concerns of care collaboration in diabetic care, patient monitoring, self-care, and patient knowledge of aspects of diabetes. Coordinated diabetes care using collaborative paradigms maintains superiority in managing diabetes. Technologies that facilitate healthcare collaborations are therefore warranted. Health information technologies that educate people with diabetes on aspects of their disease and self-care also maintain relevance in the comprehensive management of the disease as they enhance patients’ accountability in managing their disease. The reviewed articles also reinforce the need for patient monitoring and assert the significance of glucose monitoring technologies such as CGM and health information technologies such as telehealth as valuable technologies in diabetes monitoring and management.
