In Florida, an advanced practice nurse practitioner is considered an individual who has acquired a master’s degree level in a given specialty from an accredited institution in Florida, Colombia, or other states. The board of nursing must approve the nurse in Florida. The state has protocols that define the scope of advanced practice nurse practitioners, and the nurse practitioners are, therefore, expected to serve according to these guidelines and protocols. Interestingly, the Nurse Practice Act states that nurse practitioners can prescribe, administer, dispense, and even order medications (Tony-Butler & Martin, 2018). In addition, the nurses can order diagnostic investigations, coordinate healthcare services, order physical therapy and occupational therapy, and manage selected medical diseases. For uncomplicated acute illnesses, the nurse practitioner is authorized to initiate, alter, maintain and monitor treatment without physician supervision (Toney-Butler & Martin, 2018). However, for each speciality, there is a protocol to be followed. In the case where supervision is required based on the protocol, the nurse practitioner must work under a physician from the practice group.

Conversely, in California, the regulations are very strict, and the practice scope of the advanced practice nurse practitioners is limited. To begin with, the state restricts the nurse practitioner from handling patients without the physician’s supervision. Besides, nurse practitioners are not authorized to make decisions regarding patient treatment and are, therefore, expected to work in the physician’s company. Secondly, unlike in Florida, nurse practitioners outside the state of California must submit endorsements of the applications (California Board of Registered Nursing, 2022). In Florida, on the other hand, individuals with master’s degrees from a credited institution anywhere in the state are allowed to practice. Accordingly, this is very unfair since the nurse practitioners are prepared to work fully within their scope during their training. Given the differences across the states, the advanced nurse practitioner should recognize and appreciate the limitations of the practice scope across the cities (Milstead & Short, 2019). This is crucial, especially for the advanced nurse practitioner who may be willing to relocate to another state.

Sustainable Leadership, on the other hand, is focused on balancing economic, social and environmental considerations to create long-term value for all stakeholders (Elkington, 1997).

Business objectives can positively and negatively impact stakeholders, depending on how they are pursued (Carroll, 1991). For example, a company that prioritizes cost-cutting measures over employee well-being may negatively impact the health and safety of its workers (Smith, 2018). On the other hand, a company that prioritizes social responsibility and sustainability may create positive impacts, such as reducing its environmental footprint and supporting the well-being of its workers and local communities (Jones, 2019).

Similarly, sustainable Leadership can have positive and negative implications for stakeholders (Elkington, 1997). For example, a leader who prioritizes sustainability may make decisions that benefit the environment and society at the expense of the company’s financial performance, which in turn may negatively impact shareholders (Johnson, 2020). On the other hand, a leader who prioritizes sustainable development may create long-term value for the company, its shareholders, and all other stakeholders (Williams, 2021).

It is important for business leaders to consider the ethical implications and impacts of business objectives and sustainable Leadership and to engage effectively and responsibly with stakeholders to maintain competitive advantages, especially in the post-Covid-19 business environment (World Economic Forum, 2021).  Black women are subjected to disproportionate maternal health outcomes leading to high death rates related to pregnancy complications. Health system and societal factors contribute to maternal mortality and poor health outcomes among African American women. These women are more likely to experience racial discrimination and other barriers while accessing health care services (Taylor et al., 2019). Structural racism in social service and healthcare delivery results in African American women accessing poor care quality than their white counterparts. Additionally, they are not treated with respect and dignity and are denied necessary care when in pain.  Institutional racism places black women at high risk of dying from pregnancy complications. 

Background of the Issue

The United States considers high maternal mortality rates a significant public health issue. Although the government has channeled adequate resources to improve hospital-based maternity care, United States is ranked among the worst industrialized nations for pregnancy-related mortality rates (Madeline, 2020). However, from a global perspective, the country’s maternal mortality rates are low but are increasing rapidly among African American women. These women face disparity in accessing maternal care resulting in poor health outcomes. Historically, African American populations have been denied opportunities for quality and safe care.  As a result, African American women receive reduced healthcare quality and are given less attention while accessing care. The inequality is prevalent among pregnant African American women who are four times likely to succumb to pregnancy-related complications (CDC, 2019). Healthcare disparities contribute to poor quality of care and pregnancy-related complications among African American pregnant women.

System barriers, including sexism and racism, contribute to income inequality and poor health outcomes among African American women. According to the National Partnership for Women & Families (2018), Black women’s median wages are $36,227 annually. The amount is $21,698 less than white, non-Hispanic men’s median wages. Because of these income differences, African American Women and their dependents have inadequate finances to support their households. Additionally, they are forced to choose between healthcare, food, childcare, and housing because of their inadequate income. Thus, African Americans’ poor economic status affects their use of healthcare services and health outcomes.

 Limited access to insurance coverage undermines African American women’s access to quality and safe care. According to National Partnership for Women & Families (2018), African American women are underinsured because of their socioeconomic status.  These financial barriers limit African Americans’ access to necessary care, including prenatal care. Consequently, African American women suffer from preventable chronic health illnesses and medical conditions like cardiovascular disease, hypertension, and diabetes. Change Theories and Leadership Strategies

Leaders determine the success of changes in an organization. They communicate the change process; encourage people to implement the change process, and continuously update them regarding the progress made. Therefore, the theory of planned change and democratic leadership style will help meet the change objective. Democratic leadership solicits view from individuals affected by the change process to minimize their resistance (Mansaray, 2019). The emergency department employees, the primary care setting medics, and organizational leaders have a significant responsibility in ensuring that the suggested changes are integrated into the culture of the organization.

 The theory of planned change provides steps to consider when examining the issue and identifying solutions to address it. The steps are seven in number and include: diagnosing the problem; assessing the capacity and motivation for change; examining the change agent motivation and resources; designing change strategies and objectives; identifying the change agent role and responsibilities; maintaining the change; and ensuring that the proposed changes become part of organizational culture (Burke, 2017). The seven steps require excellent communication between stakeholders, regular feedback, and practical problem-solving skills. These aspects require collaboration between key stakeholders to address overcrowding in the facility’s emergency department.
